RISE-WV provides transitional housing and comprehensive support services for individuals returning to the community following incarceration, substance use treatment, or other life-altering challenges. Our team works alongside each participant to help secure identification documents, access healthcare, connect with employment and educational opportunities, develop life skills, and build a foundation for long-term independence. Through compassionate support, accountability, and community integration, we help individuals move beyond crisis and toward stable, productive lives.

Reentry & Supportive Housing

The RISE Health & Wellness Center provides accessible, compassionate healthcare services for our residents and community members. Our team works alongside patients to address primary healthcare needs, manage chronic conditions, improve overall wellness, and connect individuals with the resources necessary to achieve long-term health and stability. Through our integrated clinic and rural mobile health services, we help ensure that residents, families, and communities across West Virginia have access to quality care, preventive services, and ongoing support close to home.
